Tips to get more from it

The what-did-you-like multi-select (ease of use, design, speed, support, pricing) is your positioning research — the option chosen most is the thing your marketing should lead with. It allows Other, so surprises surface too.
One caveat before publishing: the NPS field requires a Pro plan ($14.99/mo) to publish. On the free plan, delete that single field and the other five run as-is.
Pair the star rating with the improve box when reading responses: a low rating with a specific complaint is a roadmap item, while a low rating with an empty box usually means the problem is elsewhere — pricing, expectations, or the wrong audience.

What questions should a customer feedback form ask?

One rating, one what-worked question, and one open what-could-improve question is a complete minimum viable feedback form — which is what this template is. Add questions only when a specific decision needs the answer.

Is this feedback form really free?

Yes — unlimited forms and 500 responses per month on the free plan, no card required. The one exception is the NPS field, which needs a Pro plan to publish; remove it and everything else works free.
