Tips to get more from it

The photo upload accepts up to three images at 10 MB each and is image-only, so nobody attaches a PDF by accident. Photo reviews convert browsers far better than text alone — consider mentioning that in your follow-up email.
The headline field's prompt shows an example of the punchy one-liner you want. Swap the example for one from your actual product category to steer reviewers.
The yes/no recommend question gives you a quick quality filter: publish the Yes reviews first, and read the No reviews as free product feedback.
