Tips to get more from it

Rename the matrix rows (create a project, invite a teammate, export a report) to your actual test tasks before the first session, and keep them identical across participants — the per-task columns only aggregate if the rows match.
The where-did-you-get-stuck question is required; the what-surprised-you one is not. That asymmetry is intentional — every participant hit friction somewhere, but forcing a surprise produces filler.
The 7-point ease rating is a rough single-question usability score. Track its average across test rounds and you get a poor-man's SUS trend without the ten-question overhead.

How many usability test participants do I need?

Five participants typically surface most major usability problems in a given design. Run five, fix what you find, then test again — the same form works across rounds if you keep the task list stable.

Can I change the rating scale or the tasks?

Everything is editable: the 7-point scale's range and end labels, the matrix rows and columns, and both open questions. Ids and structure stay intact, so responses keep exporting cleanly to CSV.
