Academic Research Survey template
A ready-to-run academic survey built around a four-statement Likert matrix on remote collaboration, plus role and frequency questions that give you clean cross-tabs. The follow-up opt-in is wired to a skip rule: respondents who decline are never asked for an email, which keeps the anonymous path genuinely anonymous.

The questions this template ships with — rename, reorder, or delete any of them in the builder.
When to use this template
- A graduate student collecting pilot data on how their department's lab groups coordinate across campuses
- A faculty member surveying co-authors and research staff before proposing new collaboration tooling in a grant renewal
- An education researcher comparing remote-work attitudes between undergraduates and postdocs for a methods course project
- A university IT office gathering evidence on collaboration pain points before a site-license decision
Tips to get more from it
- The matrix mixes positively and negatively worded statements ("meetings more productive" vs. "context gets lost") — keep that balance if you swap in your own items, since it guards against straight-lining
- The email field only appears after a yes on the follow-up question, thanks to the skip rule. If your IRB protocol requires full anonymity, delete both fields and the rule in one step
- Swap the role dropdown's options to match your sampling frame in the field editor — the matrix and open-ended question don't need to change
Questions people ask
Can I run a Likert-scale survey on Zunoform for free?
Yes. The matrix question, dropdowns, and open-ended fields in this template all work on the free plan, which includes unlimited forms and 500 responses per month with no card required.
How do I export survey responses for analysis in SPSS or R?
Responses export as CSV, with each matrix row as its own column. From there they import directly into SPSS, R, Excel, or any analysis tool that reads CSV.
Can respondents stay anonymous if they skip the follow-up?
Yes. The email question is skipped automatically when someone answers "No, this survey only," so no contact information is ever requested from respondents who decline the interview.
