Tips to get more from it

The matrix mixes positively and negatively worded statements ("meetings more productive" vs. "context gets lost") — keep that balance if you swap in your own items, since it guards against straight-lining
The email field only appears after a yes on the follow-up question, thanks to the skip rule. If your IRB protocol requires full anonymity, delete both fields and the rule in one step
