Screen out before you collect
A screener question with a logic rule that ends the form for people outside your criteria saves you cleaning the dataset later — and saves them five wasted minutes.
One idea per question
"Was the service fast and friendly?" can't be answered by someone who found it fast and rude. Split double-barrelled questions in two.
Put demographics last
Age and income questions feel intrusive when they open a survey and unremarkable when they close one. Ask the interesting questions while attention is highest.

Can I run an anonymous survey?
