HR form templates

HR forms carry more weight than most: they collect documents, capture signatures, and often need an audit trail. These templates handle the file uploads and signature fields that generic form builders make you bolt on.

What makes these forms work

Collect the document, not a promise to send it

Any form asking for a CV, a receipt, or an ID should have a file upload question in it. Chasing attachments by email afterwards is where HR admin time actually goes.

Signatures belong in the form

Onboarding paperwork, equipment checkout, and incident reports all need a name attached to an agreement. An e-signature field records it at submission rather than in a separate signing tool.

Keep anonymous surveys genuinely anonymous

If you promise anonymity on an engagement survey, don't ask for an email in the same form. Use conditional logic to make contact details optional and only for people who opt into follow-up.

Questions people ask

Can these HR forms collect signatures?

Yes — templates like employee onboarding and equipment checkout include an e-signature question. Signature fields require a Pro plan at $14.99/month to publish.

Can employees upload documents?

Yes. File upload questions are on the free plan, with a 10MB limit per file on free and 100MB on Pro.
