Tips to get more from it
- The matrix field renders all five competencies against four ratings in a single grid — far faster to complete than five separate rating questions, and the responses view shows the grid intact.

Questions people ask
Can I change the rating scale?
Yes. The matrix columns ('Below expectations' through 'Outstanding') and the overall-rating options are both plain editable lists — rename them, or add a fifth level.
Are review responses anonymous?
The template asks who is being reviewed but not who is writing — reviewers are as anonymous as your distribution method. Send the link without identity-collecting fields and responses carry no reviewer name.
Does the performance review template cost anything?
