Tips to get more from it

The ID upload accepts two image files at up to 10 MB each — enough for both sides of a card. Tighten or loosen the limits in the field editor if you also need visa pages.
The handbook signature is a real e-signature field, so keep the handbook itself linked in your welcome email as the description suggests — the form records acknowledgment, not the document.
The team dropdown feeds routing: turn on response notifications so IT sees each submission the moment it lands, before the start date sneaks up.

Is this onboarding form template free to use?

The template and unlimited forms are free, with 500 responses a month and no card required. Because this one includes an e-signature field, you'll need the Pro plan ($14.99/mo) to publish it with the signature step — or delete that field and publish free.

Can new hires upload their ID securely?

Yes. The file field accepts image uploads (both sides of a card, up to 10 MB per file), and responses are only visible to the form owner. You can export everything, files included, from the responses view.

Can I change the teams in the dropdown?

Every question is editable. Open the team dropdown in the builder, rewrite the options to match your org chart, and add or remove any other fields you like.

Does the e-signature count as acknowledgment of the handbook?

The signature field captures a drawn signature with the response, timestamped like the rest of the submission. Whether that satisfies your policy requirements is a question for your counsel, but it's the standard pattern for handbook acknowledgment.
