Tips to get more from it

The skip rule is already wired: choosing Vacation, Sick, Parental, or Bereavement jumps past the free-text 'what kind of leave' question. Add your own categories to the choice field and the rule keeps working.
The manager's email is collected as a field, which means the requester routes their own approval — no lookup table to maintain as the org changes.
Keep the coverage question. Approvals go faster when the manager can see a named teammate has already agreed to pick up the work.
Responses export as CSV, so reconciling requests against your PTO balance sheet at month-end is a two-minute job.

Questions people ask

How does the conditional question work?

The form has a built-in logic rule: the free-text follow-up is skipped unless the leave type is Other. You can see and edit the rule in the builder's logic panel.

Does the manager get notified automatically?

The form collects the manager's email as an answer. Turn on response notifications and the form owner is emailed each submission; forwarding to the named manager is then one click.
