Employee Offboarding template
An offboarding checklist form for departing employees: last working day, which company property is coming back, handover status, where projects and shared logins live, and a personal email for the final payslip. It turns a scattered process into one submission HR can act on.

When to use this template
- An IT manager who keeps discovering laptops and access badges are missing weeks after someone's last day
- A People Ops team that needs a personal email on file before payroll cuts the final payslip and tax forms
- A consultancy where every departure means client docs, drive folders, and shared credentials that someone must be able to find
- Any company formalizing offboarding after its first messy departure
Tips to get more from it
- The equipment checklist is a multi-select with a 'None of these' option — edit the list to match what you actually issue (monitors, corporate cards, keys) so nothing slips through.
- The handover-status question ('complete / in progress / not yet started') gives HR a triage signal at a glance. Sort responses by it in the responses view to see who needs a nudge before their last day.
- The alumni mailing list opt-in is genuinely optional — leave it that way. Forced opt-ins on an exit form set exactly the wrong tone.
- Export responses as CSV at the end of each quarter for a clean audit trail of returned equipment.
Questions people ask
When should an employee fill out an offboarding form?
As soon as the departure is confirmed — the form asks for the last working day and handover status, both of which are more useful with two weeks of runway than two days.
Can I customize the list of company property?
Yes. The returns checklist is an ordinary multi-select field — open it in the builder and swap the options for whatever your company issues.
Is this offboarding template free?
Yes. It uses no premium field types, so it publishes on the free plan — unlimited forms, 500 responses a month, no card needed.
