Tips to get more from it
- The reason-for-leaving dropdown is your trend line. Keep the options stable across the year, export to CSV quarterly, and count — three 'Management' answers from one team is a signal no single interview surfaces.
- 'What could we have done to keep you?' is deliberately optional. The employees who answer it are handing you a retention playbook; the ones who skip it are telling you something too.
- Only the reason and star rating are required — the low floor is why completion stays high with people who owe you nothing.
- The ending screen promises answers are reviewed in aggregate. Honor it: share themes with leadership, never verbatim quotes that identify the author.

When should the exit interview form be sent?
After resignation is confirmed but before the last day — most teams send it about a week out. Once someone has left, response rates fall off a cliff.
Are exit interview answers confidential?
Responses go only to the form owner, and the template's closing screen commits to aggregate review. Keeping that commitment is what keeps future leavers honest.
