Tips to get more from it
- Only 'keep doing' is required among the three open prompts — a deliberate choice. Everyone can name a strength; forcing a 'stop doing' answer generates filler. Preserve that asymmetry if you edit.
- The 'how closely do you work with them' question is your weighting key: daily collaborators' frequency ratings mean more than occasional ones. Filter by it when you compile.
- The welcome screen promises anonymization before sharing — that promise lives in your process, not the software, so compile responses into a summary rather than forwarding raw exports.
- Matrix rows are editable; swap in your company values if you'd rather measure against those than the default behaviors.

Is 360 feedback through this form anonymous?
The form never asks for the reviewer's name, so responses arrive unattributed. The form owner sees each raw response, which is why the template's welcome screen promises anonymization and aggregation before anything is shared with the subject.
How many peers should review each person?
Common practice is four to eight. On the free plan's 500 responses a month you can run a full cycle for a 60-plus-person company without paying anything.
Can I reuse the same form for every employee?
