Tips to get more from it
- This template ships with anonymous responses turned on. That setting means the API stores no IP address, user agent, referrer, geolocation, browser language, or screen size for the form, and it subtracts those from any partial row already saved — so a report left half-finished cannot leave identifiers behind either. Answers, timing, tags, and hidden fields are still stored.
- The contact-preference question drives a skip rule: pick "Keep my report anonymous" and the contact details question never appears, so nobody has to stare at a blank name field deciding whether to trust you.
- The opening statement sets out what happens next — acknowledgment within two working days, nothing shared with the subject of the report until the reporter has been spoken to. Rewrite it to your actual policy, and then meet it. A process that misses its own stated timeline is worse than no stated timeline.

Can employees report a grievance anonymously?
Yes. The contact question is skipped when the reporter chooses to stay anonymous, and the form has anonymous responses enabled, so no IP address, user agent, referrer, geolocation, browser language, or screen size is stored — including on partially completed rows.
Who can see the reports?
Only the form owner and anyone you share the workspace with — normally People Ops. The free plan includes one seat; Pro adds up to three and Business up to ten if a small panel needs access.
Is Zunoform suitable for sensitive HR reports?
Responses are private to your workspace and the anonymous setting is genuinely enforced server-side. Be clear-eyed about the limits, though: there is no SSO, no SOC 2 report, and no HIPAA BAA, so if your organization requires those controls, check with your own compliance team before you publish this form.
What should a grievance form ask for?
The category of concern, dates, location, a first-person account of what happened, witnesses, supporting evidence, and whether the matter has already been raised. This template covers each, and keeps only the account itself required so a reluctant reporter is never blocked.
