Tips to get more from it
- The has-your-manager-heard-this question drives the skip rule: answer "Not yet" and the what-did-they-say box disappears, which stops the form from implying the employee skipped a step.
- The figure is a number field, so requests sort and total in the CSV export — useful when finance needs the aggregate cost of a review round before decisions get made.
- The two long-answer questions are where the case is actually won. Keep their descriptions, which coach employees toward specific examples and named market comparators rather than general statements about hard work.
- The ending screen says decisions land in the next cycle. Replace that with your real cadence — an unanswered raise request does more damage than a declined one.

What should an employee include in a salary review request?
Time in role, when pay last changed, the specific ask and figure, concrete scope or impact changes since the last review, and market data. The form asks for each in turn so nothing important gets left out.
Who sees the request?
Responses go to the form owner — usually People Ops — and the form collects the manager's email so the request can be routed for a first response. Turn on response notifications so nothing sits unread.
