Remote Work Request template
A remote work request form covering what managers actually weigh: the arrangement type (fully remote, hybrid, or a temporary stretch), start date, workspace and time-zone details, and a plan for staying in sync with the team. A logic rule skips the end-date question when the request is permanent.

What’s inside
The questions this template ships with — rename, reorder, or delete any of them in the builder.
When to use this template
- A hybrid company formalizing case-by-case remote arrangements into a consistent, reviewable request
- An employee proposing a two-month stretch working from another country, where time-zone overlap is the whole conversation
- A People team that wants workspace details — internet quality, dedicated desk — on record before approving permanent remote status
- Post-policy-change cleanup, when everyone's informal arrangement needs a paper trail
Tips to get more from it
- The permanent-vs-temporary question drives a skip rule: permanent requests jump past 'when would it end?'. If you add arrangement types, check the rule in the logic panel still points where you want.
- The two long-text questions do the persuading — setup and sync plan. A request that names overlap hours and standup coverage approves itself; keep both prompts' descriptions, they coach employees toward approvable answers.
- The welcome screen says most requests are approved within a week. Edit that promise to whatever your process actually delivers — a stated SLA you miss is worse than none.
- Responses export to CSV, giving People Ops a live register of who works where — useful for tax and insurance questions later.
Questions people ask
What should an employee include in a remote work request?
The form asks for the essentials reviewers look for: location and time zone, internet reliability, whether there's a dedicated workspace, and a concrete plan for overlap hours and team communication.
Why does the end-date question sometimes not appear?
A built-in logic rule skips it when the arrangement is marked permanent. Pick temporary and the end date comes back.
Can I adapt this for a hybrid schedule policy?
Yes — every field is editable. Many teams add a checkbox question for which weekdays will be remote and route the form to the manager for sign-off.
