Tips to get more from it
- The 'which session did you attend' field is what lets one form cover your whole training calendar — filter responses by session name instead of cloning the form for every workshop.
- Pace is a three-option choice (too slow / just right / too fast), not a scale — deliberately. Facilitators can act on 'too fast'; they can't act on '3.4 out of 5.'
- The relevance question uses a rating scale separate from the overall stars. When stars are high but relevance is low, you have an entertaining session that isn't changing anyone's work — a pattern averages hide.
- Send the form within an hour of the session ending. Same-day response rates dwarf next-day ones, and the 'most useful thing you learned' answers are sharper.
Questions people ask
Should training feedback be anonymous?
This template collects no names, and honest pace-and-relevance feedback is the point. If you need attendance tracking, do it separately from feedback.
Can I use one form for multiple training sessions?
Yes — attendees name their session in the first question, and you filter the responses view by session. One link on the calendar invite covers everything.
How do I share results with the facilitator?
Export the session's responses as CSV, or summarize from the responses view. The template's ending screen tells attendees feedback is shared after each cohort, which nudges answers toward constructive.
