Training Feedback template

A training feedback form that takes two minutes and tells you three things that matter: how the session rated overall, whether the content was relevant to real work, and whether the pace landed. A would-you-recommend question gives you a clean signal for deciding which sessions to run again.

Questions people ask

Should training feedback be anonymous?

This template collects no names, and honest pace-and-relevance feedback is the point. If you need attendance tracking, do it separately from feedback.

Can I use one form for multiple training sessions?

Yes — attendees name their session in the first question, and you filter the responses view by session. One link on the calendar invite covers everything.

How do I share results with the facilitator?

Export the session's responses as CSV, or summarize from the responses view. The template's ending screen tells attendees feedback is shared after each cohort, which nudges answers toward constructive.
