Tips to get more from it

The QC question is the logic hinge: "Yes" skips the anomaly description, "No" requires the collector to explain the failure while they're still standing at the site
Sample count is a number field, so exports stay numeric and sum cleanly — resist the urge to switch it to free text
The file field accepts site photos and instrument readings; submissions with visual evidence settle data-quality disputes months later
Collector ID is free text with an example format (FC-014). If your team is small, a dropdown of known IDs prevents typos that fragment your dataset

Does this form work on a phone in the field?
