Focus Group Screener template
A screener template that qualifies focus group participants and disqualifies gracefully. Two jump rules do the gatekeeping: anyone who hasn't ordered a meal kit in six months, or who is under 18, is routed to a polite "not this time" ending instead of wasting five minutes on questions that won't count.

The questions this template ships with — rename, reorder, or delete any of them in the builder.
When to use this template
- A UX research agency recruiting for a paid 60-minute session on meal-kit services — the template's shipped scenario
- A consumer-brand insights team screening for category buyers before a taste-test panel, where non-buyers must be excluded early
- A university lab that needs adult participants only and wants under-18 respondents exited politely, without collecting their contact details
- A market researcher filling four specific session slots and needing availability captured up front
Tips to get more from it
- The disqualifying jump rules fire before the contact question, so you never collect personal details from people you can't use — worth preserving if you edit the flow
- Session times display as a two-column grid on the multi-select; edit the four options to your real dates and the grid layout carries over
- There are two endings in this form: qualified respondents see the confirmation, screened-out respondents see the softer "future studies" message. Edit both — the rejection copy does recruiting work for your next study
Questions people ask
How do I automatically disqualify screener respondents?
This template uses jump rules: a "No" on the purchase question or an "Under 18" age selection routes straight to a polite ending. You can add more disqualifiers by pointing new rules at the same ending.
Do disqualified respondents still count as responses?
Yes, every submission counts toward your monthly total, which is 500 on the free plan. Their answers are recorded too, so you can see how many people screened out and why.
Can I see which session times filled up fastest?
Each submission records the slots the respondent selected, and the CSV export lists them per response — filter the column to count interest per session.
