Consulting Quote Request template
Consulting proposals stall on two unknowns: what the engagement actually is, and whether the money exists. This form settles both. It captures engagement type from one-off advisory through fractional roles, an open problem statement, the functional areas involved, expected duration, and budget approval status — showing the budget-range question only to prospects whose budget is approved or in progress.

When to use this template
- Independent consultants and fractional executives qualifying inbound work before spending an hour on a call
- Boutique consultancies routing requests by practice area, using the areas checkbox to reach the right partner
- Training and workshop providers, where the engagement-type question separates a one-day session from a six-month program
- Any advisory business where unbudgeted inquiries have been consuming proposal-writing time
Tips to get more from it
- Budget approval status is a sharper qualifier than budget size. "Approved" versus "gathering numbers" tells you where the deal really is, and the range question only appears for the first two answers.
- The problem question asks what's happening now, what should be happening, and what's already been tried. That three-part prompt is why the answers are usable instead of one vague line.
- The engagement-type question is your pricing model in disguise: advisory sessions, fixed-scope projects, retainers, and interim roles are four different quotes.
- The ending promises a proposal within five business days. Set a cadence you'll hold — on a form about professional advice, a missed self-imposed deadline is a poor first impression.
Questions people ask
What should a consulting inquiry form ask?
Company and industry, size, engagement type, the problem in the client's own words, functional areas, expected duration, budget status, and start date. This template covers all of them in thirteen steps.
How do I ask about budget without scaring prospects off?
Ask about approval status before amount, as this template does. "Has budget been approved?" is easier to answer honestly than a range, and the range question is skipped for anyone still gathering numbers.
Is this template free to use?
Yes — the free plan covers unlimited forms, 500 responses a month, and the conditional logic this template uses. Responses export as CSV whenever you want them in a spreadsheet.
