Donation Form template
A donation form with five giving tiers from $10 to $250, an optional dedication step, and card payment built in. The payment amount is wired to the tier the donor picks, so the charge always matches their choice without any manual math. Donations go straight to your own Stripe or PayPal account — Zunoform never takes a cut.

What’s inside
The questions this template ships with — rename, reorder, or delete any of them in the builder.
When to use this template
- A community animal shelter running its year-end appeal and mailing donor letters in January
- A school PTA collecting gifts for a new playground, where donors often give in someone's honor
- A church or mosque replacing the physical collection basket with a link in the weekly newsletter
- A local theater troupe funding its next production with tiered supporter levels
Tips to get more from it
- The tier prices live in the payment question's price source. Change "$25 — Friend" to "$35 — Friend" in the field editor and update its mapped price, and the charged amount follows automatically.
- The dedication question drives a skip rule: donors who answer "No" never see the honoree name question, so the quick path stays quick.
- Rename the tiers to match your campaign — "Plant a tree", "Plant ten" — without touching anything else; only the price mapping needs to agree with the option labels.
- Export responses as CSV at year end to build your donor acknowledgment list, honoree names included.
Questions people ask
Does Zunoform take a percentage of donations?
No. Payments run through your own Stripe or PayPal account using your keys, so donations land directly with you minus only the processor's standard fees. Zunoform charges a flat $14.99/mo for the Pro plan, which is required to publish forms with a payment field.
Can donors choose their own amount instead of a tier?
This template maps fixed tiers to fixed prices. You can add or edit tiers freely — each option just needs a price in the payment question's mapping. Every question in the template is editable after you start from it.
Do donors get a receipt?
The form collects an email on the contact step, and Stripe or PayPal sends its own payment receipt. You can also enable Zunoform's response notifications so your team is pinged for every gift.
