What makes these forms work
Suggest donation amounts
A blank amount field gets small, hesitant answers. Preset tiers with a label — supporter, sustainer, patron — raise the average because they tell people what normal looks like.
Waivers need the agreement visible
Put the terms in a statement block immediately above the signature, not behind a link. A signature under text nobody saw is worth less if it's ever questioned.
Screen applications with logic
Rental, adoption, and volunteer applications all have disqualifying answers. Branch on them early so unsuitable applicants don't spend ten minutes on a form you'll decline.
Questions people ask
Can I take donations through a form?
Yes. Map an amount to each donation tier and the payment field charges it through your own Stripe or PayPal account — we take no percentage. Payments need the $14.99 Pro plan.
Are e-signatures on waivers legally binding?
Electronic signatures are broadly recognized in many jurisdictions, but whether a specific waiver is enforceable depends on your local law and the wording. Have a lawyer review anything with real liability attached.
