Tips to get more from it

The health question ends with "None of these apply to me", and a skip rule uses that answer to hide the follow-up detail question. Add conditions to that list freely — the rule keys off the last option, not the ones above it.
The confirmation checklist requires all four boxes, including the one about spelling and placement. That single line is the cheapest insurance a studio has against a redo argument.
The risk statement is an ordinary statement field. Replace it with the wording your studio or your attorney prefers; nothing else in the form depends on its text.
Keep the date-of-birth field required and check it against photo ID in person. The form records what the client entered — it does not verify age on its own.
