Tips to get more from it

Plan prices live in the payment question's price source, keyed to the option labels. Change "Full access — $49/mo" and its mapped amount together so the label and the charge never disagree.
The $25 joining fee is the payment field's base amount, added on top of the selected plan. Set it to 0 if you run a no-joining-fee promotion — the plan prices keep working.
The health screening is a three-way choice (No / Yes / I'm not sure), and only "No" skips the detail question. "I'm not sure" deliberately lands in the follow-up, because that answer is the one worth a conversation.
Totals are recomputed on the server when the payment is taken, so a respondent cannot edit the amount in their browser and pay less than the plan costs.

Questions people ask

Does Zunoform take a cut of membership payments?

No. Payments run on your own Stripe or PayPal keys, so money lands in your account minus only your processor's standard fees. Zunoform charges a flat $14.99/mo for Pro, which is what publishing a form with a payment field requires.

Can the form charge a different amount per plan?

Yes, and that is how this template is built. Each plan option maps to its own price on the payment question, and the total follows the applicant's selection with the joining fee added on top.

Can I collect recurring monthly dues through the form?

The payment field takes a one-time charge — here, the first payment plus the joining fee. Set up the recurring billing in your own Stripe or PayPal account using the details the form collected.
