Tips to get more from it

The uses question is a multi-select, so consent is granular — someone can allow your website but not press coverage, and the response records exactly that split.
The minor's-name field is gated by a skip rule on the "Who is this consent for?" question; adults signing for themselves get a shorter form automatically.
The signature description states that consent can be revoked in writing — keep a line like that, and note revocations against the exported record.
Single-page layout is deliberate here: consent forms read better when the statement, options, and signature are visible together.
