- The uses question is a multi-select, so consent is granular — someone can allow your website but not press coverage, and the response records exactly that split.
- The minor's-name field is gated by a skip rule on the "Who is this consent for?" question; adults signing for themselves get a shorter form automatically.
- The signature description states that consent can be revoked in writing — keep a line like that, and note revocations against the exported record.
- Single-page layout is deliberate here: consent forms read better when the statement, options, and signature are visible together.

Yes. The form asks who the consent is for, and choosing "A minor in my care" adds a required field for the minor's full name, with the adult's contact details and signature on the same record.
