Food Delivery Order template

An order form for a small food business with a six-item menu, live per-item pricing, a delivery-or-pickup choice, and card payment at the end. Each menu selection is mapped to a price, so the charge equals exactly what was ticked — and pickup customers skip the delivery-address question entirely. Payments go to your own Stripe or PayPal account.

Questions people ask

Do I pay commission on orders like on delivery apps?

No. Payments run through your own Stripe or PayPal keys and Zunoform takes no percentage — you pay the flat $14.99/mo Pro plan (required to publish payment forms) plus your processor's standard fees. For a business doing more than a few orders a week, that is far cheaper than marketplace commissions.

Does the total update as customers pick items?

Yes. Each menu option is mapped to a price on the payment question, and the amount charged reflects the exact combination selected.

Can customers order multiple quantities of one item?

The menu is a multi-select, so each item is picked once per order. For quantities, add a number question per popular item or have customers submit the form once per duplicate item — or split large items into options like "2x Garlic knots — $12".
