Equipment Checkout template
An equipment checkout form for the shared gear pool: what's needed, pickup and return dates, what it's for, and an e-signature accepting responsibility for the kit. That signature is the point — it changes how carefully the camera comes back.

The questions this template ships with — rename, reorder, or delete any of them in the builder.
When to use this template
- A marketing team sharing one camera kit, two projectors, and a tangle of adapters that historically vanish before all-hands week
- A university media lab lending equipment to students, where the responsibility signature is the difference between a loan and a gift
- An IT desk managing loaner laptops for visitors and hardware-repair gaps
- A film or events crew reserving kit for shoots, using the purpose field to referee conflicts over the same gear
Tips to get more from it
- The gear dropdown ships with common items and allows an Other write-in — edit the options to your actual inventory so requests map to things you can hand over.
- Pickup and return dates are both required, which means every response is a complete loan record; export the CSV and you have an audit trail of who had what, when.
- The 'what's it for' question earns its place the day two teams want the camera kit for the same Friday — the answer is how you prioritize.
- The signature description sets the damage-reporting window at 24 hours. Adjust it to your policy; the field records the drawn signature with the response.
Questions people ask
Does the checkout signature actually matter?
It captures a drawn e-signature, timestamped with the response — a clear record that this person accepted responsibility for this equipment on this date. Enforcement is your policy's job, but the record removes the ambiguity.
Is a plan required to use the signature field?
You can build with it free, but publishing a form containing an e-signature requires Pro ($14.99/mo). If you want to stay on the free plan, swap the signature for a required 'I accept responsibility' checkbox.
Can I track what's currently checked out?
The responses view shows every reservation with pickup and return dates; export to CSV for a sortable loan ledger. The form doesn't block double-booking, so glance at responses before confirming contested gear.
