Maintenance Request template
A maintenance request form that gets facilities the four things they need before rolling a cart: exact location, issue type, an honest urgency triage, and a photo. The three-level urgency question — emergency, blocking, routine — is what turns a request inbox into a work queue.

When to use this template
- An office manager fielding 'the tap in the third-floor kitchen is dripping again' messages across email, Slack, and hallway ambushes
- A property manager giving tenants one link — printed as a QR code by the elevator — for every building issue
- A school where teachers report broken desks, flickering lights, and radiator problems to a two-person maintenance crew
- A gym or coworking space where members spot issues staff won't see until closing
Tips to get more from it
- The location field's description shows the format you want ('HQ, 3rd floor, Room 3.14') — keep an example in there. Vague locations are the single biggest time sink in facilities work.
- The photo upload takes up to three images. A picture of the leak answers half the diagnostic questions before anyone walks over; nudge reporters toward it even though it's optional.
- The urgency options are worded to self-triage ('Emergency — safety hazard' vs 'Routine — when you can'). Resist collapsing them to High/Medium/Low; descriptive labels get truthful answers.
- The ending screen states response times — emergencies immediately, routine within two business days. Edit those to your real SLAs, then turn on response notifications so emergencies actually reach someone fast.
Questions people ask
How do maintenance requests reach the facilities team?
Every submission lands in the form's responses view, and with notifications on, the owner gets an email per request. Sort by the urgency answer to work the queue top-down.
Can tenants or employees attach photos of the problem?
Yes — the form accepts up to three images at 10 MB each, straight from a phone camera.
Is this maintenance form free to run?
Yes. No premium fields, so it publishes on the free plan — unlimited forms, 500 requests a month, no card. That's a lot of dripping taps.
