Operations and IT form templates

Ops forms exist to turn scattered requests into a queue. These templates ask the triage questions up front — location, urgency, category — so whoever picks the request up can act without a round of clarifying messages.

What makes these forms work

Make urgency self-describing

"Emergency — safety hazard" and "Routine — when you can" get honest answers. High/Medium/Low gets everything marked High.

Ask for a photo

One picture of the leak, the error screen, or the damaged equipment answers half the diagnostic questions before anyone walks over.

Publish your response times on the ending page

The last screen is the one people remember. Stating what happens next, and by when, cuts the follow-up messages asking whether anyone saw it.

Questions people ask

Can requests go straight to Slack?

Yes — the Slack integration posts each submission to the channel you choose, formatted for triage. Slack is on the Pro plan; Google Sheets and webhooks are free alternatives.

Can I build an inspection checklist?

Yes. The matrix question type gives you rows of checks against Pass/Fail/N-A columns in a single question, which is how the safety inspection template works.
