How does the conditional logic work?
When “Does this replace existing equipment?” is answered “No”, the form skips the intervening follow-up and continues at “How will this equipment support your work?”. Other answers keep the follow-up visible. Test both paths after changing choices or moving questions.
Does submitting approve the purchase?
No. It submits a request with its justification. Budget approval, inventory checks and shipping remain separate steps.
Can I change the logo, imagery and ending?
