Office Move Request template
An office move request form for desk changes, floor moves, and team relocations: who's moving and how many, where from and where to, preferred date, what needs moving or setting up, and any specific setup requirements. Solo movers skip the headcount question, and standard-desk requests skip the setup detail.

When to use this template
- A facilities team running weekly move batches and needing requests to arrive complete rather than as a hallway conversation
- A growing company reshuffling floors quarterly, where the reason field decides which team gets the window side
- An office manager coordinating accessibility and sit-stand desk requirements, captured in writing rather than remembered
- A workplace team moving a whole department, where knowing whether monitors, docks, and storage come along changes the crew size
Tips to get more from it
- Two skip rules keep this short for the common case: 'Just me' hides the headcount question, and 'a standard desk is fine' hides the setup detail. A one-person seat change is nine questions, not twelve.
- The from-and-to fields both ask for building, floor, and desk numbers. Vague locations are the single largest source of move-day confusion, so the descriptions are doing real work.
- The what-needs-moving checklist is required and it's the crew's work order — desks and chairs is a different job from a seat change with nothing to carry.
- The ending states a Wednesday cutoff for the following week's batch. Change it to your real cadence; a stated schedule is what stops the follow-up messages.
Questions people ask
Why did the form skip the headcount question?
A logic rule hides it when you answer 'Just me' — there's nothing to count. Choose 'A few people' or 'My whole team' and it appears.
Can people request accessibility or sit-stand setups?
Yes. Answering 'Yes' to the specific-setup question opens a free-text field, and the prompt names sit-stand desks, step-free routes, extra monitors, and quieter spots so people know it's a fair thing to ask for.
Is the office move request template free?
Yes — no premium fields, so it publishes on the free plan with 500 responses a month included.
