Inventory Request template
An inventory request form for office and team supplies, kept to seven fields on a single page: who's asking, which team, an items-and-quantities list, urgency, and a delivery spot. Requests batch into the weekly Friday order, which is the whole discipline — one order run, not thirty ad-hoc ones.

When to use this template
- An office manager consolidating 'we're out of coffee / notebooks / HDMI cables' pings into one Friday procurement run
- A front desk restocking against requests from six teams, each tagged by the team dropdown
- A lab or workshop where consumables run out mid-week and the urgency question decides what jumps the batch
- A multi-floor office using the delivery-spot field so supplies land at the right kitchen, not the loading dock
Tips to get more from it
- The items field asks for one item per line with quantities ('Notebooks × 20') — that formatting convention is what lets you skim ten requests and build one consolidated order. Keep the example in the description.
- Urgency is framed around your order cycle ('whenever the next order goes out'), not abstract priority — requesters self-sort honestly because the middle option costs them nothing.
- This template uses the single-page layout on purpose: a supplies request is a 30-second errand, and everything visible at once respects that.
- The ending screen tells requesters stock orders go out every Friday. If your cadence is different, change the line — a stated schedule is what stops the 'any update on my notebooks?' follow-ups.
Questions people ask
How do batched supply orders work with this form?
Requests accumulate in the responses view all week; before the Friday order, you read down the items column — or export CSV — and consolidate quantities into one purchase.
Can teams request anything, or a fixed catalog?
As shipped, the items field is free text, which suits general supplies. For a controlled catalog, swap it for a multi-select of approved items in the builder.
Is this template free for internal use?
Yes — no premium fields, unlimited forms, and 500 responses a month free. An office's supply requests rarely dent that.
