Admission Application template
A private-school admission application covering the full first-round file: applicant contact, date of birth, address, program choice, current school, and a 300–500 word personal essay. The welcome screen tells families to allow 10–15 minutes and gather details first, because the form is honest about there being no draft-saving.

What’s inside
The questions this template ships with — rename, reorder, or delete any of them in the builder.
When to use this template
- A private high school or academy running fall admissions with several program concentrations
- An international school collecting applications across general studies, STEM, arts, and IB tracks
- A selective middle school program that reads a short essay from every applicant
- A charter school gathering structured applicant data ahead of an interview round
Tips to get more from it
- The essay prompt — "what do you hope your next four years look like?" — is forward-looking on purpose; it produces more distinct answers than achievement recaps. Adjust the word range in the description, not just the label.
- Date of birth and address use dedicated date and address fields, so exports arrive structured for your admissions system rather than as free text.
- "How did you hear about us?" has "Other" enabled, and its write-ins are your admissions marketing report — school fairs versus current families versus social media.
- The welcome screen warns that drafts can't be saved; keep that warning if you edit the copy, since families abandoning mid-essay is the failure mode to avoid.
Questions people ask
Can applicants attach transcripts or report cards?
This template collects the written application; add a file upload question if your first round needs documents, or request them at the interview stage as the ending screen implies.
Can I charge an application fee in the form?
Yes — add a payment field connected to your own Stripe or PayPal account. Payment fields require the Pro plan ($14.99/mo) to publish, and Zunoform takes no cut of the fee.
How does the admissions team work through submissions?
Each application is a response in the dashboard, and the structured fields — program, school, date of birth — export as CSV for your review spreadsheet or SIS import.
