Tips to get more from it

The medical question drives a skip rule — answering "No" hides the follow-up long-answer field, so only families with allergies or conditions write details.
Date of birth uses a real date field and the address uses a structured address field, which keeps the CSV export clean for your student information system.
The grade dropdown ships as Kindergarten through Grade 5 — extend it in the field editor for a K–8 or K–12 school.
The welcome screen tells parents what to have on hand (date of birth, emergency contact) before they start; keep that habit when you edit the copy, since it noticeably reduces abandoned forms.
