Tips to get more from it

The matrix covers five aspects of the course on one screen — students finish in the promised five questions, which is why completion stays high. Add rows sparingly.
The course dropdown lets one form serve a whole department; filter the CSV export by that column to build per-course reports.
"Yes, with caveats" as a middle recommendation option captures the honest lukewarm answer that a plain yes/no forces into a false positive.
The single change question ("what would most improve the course?") produces more usable feedback than a general comments box — one concrete suggestion per student.

Questions people ask

Are the evaluations really anonymous?

Yes — the template collects no identifying fields. If you need section-level detail without identifying students, add a section dropdown rather than a name field.

How do instructors see their results?

Responses can be viewed in the dashboard or exported as CSV, filtered by the course dropdown, and shared after grades post — the workflow the template's ending screen describes.
