Alumni Survey template
An alumni survey that maps where graduates landed and pulls them back into the community in the same pass: graduation year, current role, degree relevance, an NPS score for the school, engagement preferences, and a mentorship opt-in. Say yes to mentoring and a follow-up email question appears; say anything else and it stays hidden.

What’s inside
The questions this template ships with — rename, reorder, or delete any of them in the builder.
When to use this template
- A university career-services office measuring degree-to-job relevance across graduating classes
- A high school building its first alumni database ahead of a milestone reunion
- A bootcamp collecting outcome data — roles and employers — for its results page
- An MBA program recruiting mentors and guest speakers from its alumni base
Tips to get more from it
- The mentorship question drives a skip rule: only "Yes, sign me up" reveals the email field for the mentorship program, so everyone else finishes one question sooner.
- Graduation year is a number field — exports sort chronologically, letting you compare outcomes and sentiment by class.
- The degree-relevance question ("Directly related" to "Not related") is the outcome stat boards ask for; cross it with graduation year in the CSV export for a trend line.
- The NPS question benchmarks alumni sentiment year over year, but note NPS fields need the Pro plan to publish — on the free plan, swap it for a 0–10 choice question.
Questions people ask
How do I get alumni to actually fill this out?
The welcome screen promises five minutes and says the answers shape career services — a concrete reason to respond. Sending it alongside reunion announcements, as the ending screen hints, also lifts response rates.
Can I run this survey on the free plan?
Almost — the NPS field requires Pro ($14.99/mo) to publish. Replace it with a standard choice question scaled 0–10 and everything else in the template is free.
How is the mentorship interest followed up?
Alumni who opt in are asked for their best email in a field only they see. Filter the CSV export by the mentorship column and you have your mentor outreach list.
