Should we ask about background checks on a signup form?

Asking whether a check is already on file, as this template does, tells the coordinator who can be scheduled immediately. Run the check itself through your district's own process — do not collect ID numbers or check results in a form.

How do we turn responses into a volunteer schedule?

Export as CSV and filter by the interest and availability columns. Each row already carries a phone number for day-of contact and the child's class for reference.
