Tips to get more from it

The background check question drives the logic: parents who answer yes skip straight past the clearance explainer, while everyone else gets the practical steps at the exact moment they need them.
Availability and interest are both multi-selects, so the CSV export becomes a filterable roster — pull everyone free on weekday mornings who ticked classroom reading and you have your list.
The skills question surfaces the parent who is a nurse, a carpenter, or a native speaker of a language you teach. Those answers are usually more valuable than the checkbox list above them.
The clearance statement names a two-week turnaround and a free packet at the front desk. Replace both with your district's actual process before publishing.

Should we ask about background checks on a signup form?

Asking whether a check is already on file, as this template does, tells the coordinator who can be scheduled immediately. Run the check itself through your district's own process — do not collect ID numbers or check results in a form.

How do we turn responses into a volunteer schedule?

Export as CSV and filter by the interest and availability columns. Each row already carries a phone number for day-of contact and the child's class for reference.

Is this template free?
