School Application template
A school application form for families applying for a place: student name and date of birth, the grade they should enter, the term they would start, guardian contact, home address for catchment checks, current school, and an optional report card upload. A sibling question branches open only for families who already have a child at the school.

The questions this template ships with — rename, reorder, or delete any of them in the builder.
When to use this template
- A K–8 school running an application round and needing every family's details in the same shape
- An independent or faith school where sibling priority affects the offer order
- A charter school collecting catchment-relevant addresses before a placement lottery
- An admissions office replacing a downloadable PDF that arrives half-completed by email
Tips to get more from it
- The sibling question drives the one logic rule — answer no and the sibling name field never appears, which is most applicants. Answer yes and admissions gets the name it needs to check priority.
- Date of birth is a real date field and the home address is a structured address field, so the export lines up with your student information system instead of arriving as free text.
- The report card upload is optional at application stage on purpose, and accepts up to two files at 10 MB — enough for a scanned two-page report or two phone photos.
- The "anything else we should know" question is where learning support and medical needs surface early. Read those before the visit, not after the offer.
Questions people ask
How is this different from an enrollment form?
This one applies for a place: grade, start term, catchment address, sibling priority, and current school. Enrollment comes after an offer and collects the operational detail — emergency contacts, medical plans, and consent.
Can we ask for documents with the application?
Yes. The report card upload is built in, and you can add more file questions for proof of address or previous school records — file uploads publish on the free plan.
Is this application form free to publish?
Yes. No Pro question types are used, so it runs on the free plan with unlimited forms and 500 responses per month — enough for most application rounds. If you need to charge an application fee, a payment field on your own Stripe or PayPal keys requires Pro at $14.99/mo, with no cut taken by Zunoform.
