Teacher Feedback Survey template
An anonymous student feedback survey written in a teacher's own voice — "help me teach you better." One agreement matrix covers clarity, help access, fair grading, engagement, and pace; then a star rating and the paired questions that make feedback actionable: what should stay exactly the same, and what would you change.

What’s inside
The questions this template ships with — rename, reorder, or delete any of them in the builder.
When to use this template
- A teacher checking in mid-semester, while there is still time to adjust the course
- A department piloting student feedback across several sections using the class dropdown
- A new teacher gathering a baseline read in the first month
- A homeschool co-op or tutoring group asking students how sessions are landing
Tips to get more from it
- The keep/change question pair is the template's engine — "what should stay exactly the same" surfaces what is working so you don't accidentally change it, and it makes the criticism in the second answer easier to act on.
- The matrix statements are written from the student's side ("I can get help when I need it"), which draws more honest agreement ratings than teacher-centric phrasing — keep that framing when you edit rows.
- The class dropdown separates sections without identifying anyone; filter the CSV export by class to compare how Period 2 and Period 7 experience the same course.
- The ending promises to share back what you heard and what you're changing — following through on that is what earns honest answers next time you ask.
Questions people ask
Can students be identified from their responses?
No — the template collects no name or email, only which class the student is in. Anonymity is the point, and the welcome screen says so explicitly.
When during the term should I run this survey?
Mid-term beats end-of-term: the current students benefit from the changes. Many teachers run it twice and compare matrix scores across the two rounds.
Is this survey free to run?
Yes. Matrix, star, and open-answer questions all publish on the free plan — unlimited forms, 500 responses per month, no card needed.
