Case Study Request template
Most case studies die in the asking. This form makes the ask concrete: it offers three ways to take part, collects the before-and-after story in the customer's own words, and — the part that saves weeks — asks up front exactly what you're cleared to publish, from company name to specific metrics to screenshots. Customers who decline jump straight to the ending rather than being walked through questions they've opted out of.

The questions this template ships with — rename, reorder, or delete any of them in the builder.
When to use this template
- A B2B software company running a quarterly case study program instead of chasing one-off favors over email
- Agencies collecting client results at the end of a successful engagement, while the numbers are fresh and goodwill is high
- Customer success teams turning renewal conversations into published proof, with the approval question flagging legal review early
- Any marketing team whose case study pipeline stalls because nobody knows which logos and figures are actually approved
Tips to get more from it
- The publishing-permissions checkbox is the field that saves weeks. Getting name, logo, metric, and screenshot clearance in the same submission as the story avoids a second approval round.
- Customers who answer "Not right now" jump straight to the ending — a jump rule on the participation question, editable in the Logic tab. Don't make a decline sit through eight more questions.
- The results question explicitly asks for numbers and names the kinds that work: hours saved, response rates, revenue. Vague answers here are the difference between a case study and a testimonial.
- The approval question tells you on day one whether legal review is coming, which is the main reason case studies slip their publication date.
Questions people ask
How do you ask a customer to be a case study?
Make the ask small and specific: name the time commitment, offer a format choice, and promise approval rights. This template does all three on the welcome screen, and the anonymized option keeps customers in who can't be named.
What should a case study request form ask?
Contact details and title, willingness and preferred format, the before-and-after story, publishing permissions, and whether internal approval is needed. That's this template's structure.
Can customers take part anonymously?
Yes — "Yes, but keep us anonymous" is one of the participation options, and the permissions checklist lets a customer clear metrics while withholding their company name.
