Website Design Questionnaire template
Use this website design questionnaire once a project needs a concrete discovery brief. It asks who the site serves, which pages and features belong in scope, who supplies the content and what must be ready for launch. The existing-site question skips the URL follow-up for a new build.

When to use this template
- Preparing a website kickoff after an initial scope discussion
- Separating a redesign's preservation requirements from new features
- Coordinating copy, imagery and approvals across a client's team
Tips to get more from it
- Replace the feature choices with the services you actually deliver; selected features are requests for discussion, not a signed scope.
- Ask about URLs that must survive a redesign before agreeing a migration plan.
- Treat the launch date as a target and confirm content, accessibility review and approvals against it.
Use the same questionnaire for redesign discovery
For a redesign, start with the problem that needs to change: customers cannot find a service, content is difficult to maintain, or a booking step fails on a common device. Add the available evidence and its owner to the success answer. A general request for a modern look is a design preference; it does not establish how you will judge the result.
Use the carry-over question to identify important URLs, downloadable files, content and connected services. Name who will review URL mapping and redirects. Arrange analytics access through the account owner rather than collecting credentials in the brief. For a new website, the No branch skips the existing-site link and the client can write New site in the preservation question.
Turn discovery into a reviewable delivery plan
Read scope, content ownership and target date together. If the client wants new service pages but has no approved copy, record the content dependency before promising a launch. If the project changes an existing enquiry or booking flow, identify who will test it and what the receiving team needs to operate it.
Produce a short kickoff record: the user problem, included pages and features, materials to preserve, named decisions and open dependencies. Use the content collection form for actual page materials and the change request form for work added later. This keeps discovery focused while the delivery record grows with the project.
Example response and next steps
Illustrative example, not a real customer submission.
A local bakery is replacing a brochure website with a clearer way to request celebration cakes.
- Who will use the website, and what should they accomplish?
- Local customers should compare cake sizes, see collection details and request a custom cake.
- Who will supply and approve copy, images and brand assets?
- The owner approves copy; the photographer supplies new product images two weeks before launch.
How to use the response
Clarify whether the enquiry flow needs payment or only a request. Record the photography dependency and agree which existing service URLs should be retained before preparing the design scope.
Before publishing your version
Test both existing-website answers, edit the page choices and confirm that the ending explains your team's next review step. Do not use the questionnaire as a substitute for a project agreement.
Questions people ask
How is this different from a web design quote request?
A quote request helps estimate whether a project fits your services and budget. This questionnaire goes deeper into content ownership, preservation requirements and launch dependencies once discovery is appropriate.
Does this questionnaire build the website?
It creates an editable form for collecting a brief. Your team reviews the responses and builds the website separately.
Can I use it on Free?
Yes. Its included fields and conditional logic work on Free with 500 responses per month. Edit the questions and choose conversational, single-page or document layout.
