What to get right

Make urgency self-describing

High, Medium and Low gets everything marked High. "Safety hazard, needs attention today" and "Routine, whenever you get to it" get honest answers, because people can see which one describes their situation. Write the levels as sentences and the queue sorts itself.

Ask for a photo before you ask for a description

One picture of the leak, the error screen or the damaged equipment answers half the diagnostic questions before anyone walks over. File uploads are on the free plan, so this costs nothing but a question.

Route by answer, and say what happens next

Can I build an inspection checklist in a single question?

Can tickets go straight into Slack or a spreadsheet?

Can staff book a room or a piece of equipment?
