Forms for HR teams
Most HR forms are paperwork with a deadline attached: a new hire who needs to be set up on Monday, a leave request the manager has not seen, a review cycle that closes on Friday. The templates below cover that year, and the notes underneath are about the two things HR forms get wrong most often — documents that arrive separately from the form, and surveys promised as anonymous that are not.
8 templates · free plan includes 500 responses a month · no card
Contact details, role, experience, skills, and a resume upload.
New-hire details, start date, equipment sizing, ID upload, and a handbook signature.
Dates, leave type, coverage notes, and manager approval.
Category, amount, and receipt uploads — paid out with the next payroll run.
Competency ratings, biggest wins, focus areas, and an overall rating.
eNPS, satisfaction score, what's working, and an optional follow-up opt-in.
Reason for leaving, what could have kept them, and honest advice for leadership.
Weekly hours by project, an overtime branch, and notes for the approver.
What to get right
Collect the document in the form, not in a reply
A CV, a signed policy, a receipt, a copy of an ID — if the form asks for it, it should accept it. File uploads are a question type on the free plan, and the file arrives attached to the response rather than in a separate email you have to match up later. That matching is where HR admin time actually goes.
Anonymous has to mean anonymous
An engagement survey that logs IP addresses is not anonymous, and staff assume it is not. Zunoform has an anonymous responses setting that stores no IP address, user agent, referrer, geolocation, language or screen size, and strips those identifiers from partial rows too. Turn it on before you send the survey, not after someone asks.
One form per process, branched — not one form per case
Leave types, review levels, offboarding paths: these are branches, not separate forms. Conditional logic is on every plan including free, so one leave request form can ask about coverage only for absences over a week. You maintain one form and read one response table.
Questions people ask
Can HR forms capture a signature?
Yes, on the $14.99 Pro plan. E-signature is one of three Pro-only question types, along with NPS and payment, and the gate applies at publish rather than while you build — so you can lay out an onboarding pack on the free plan and see the whole thing before paying. The signature is captured with the response and its timestamp. It does not produce a countersigned PDF; if you need that, a document tool belongs downstream.
Is Zunoform suitable for employee data?
For ordinary HR administration, yes. For anything with a formal compliance requirement, check the gaps first: Zunoform does not offer a HIPAA Business Associate Agreement, single sign-on, or SOC 2. If your policy requires SSO for internal tools, that is a genuine blocker and we would rather say so here than after you have built the forms.
Can managers get notified when a request comes in?
Email notifications are on the free plan and can go to you, a shared team address, or the person who submitted. Slack notifications are on the $14.99 Pro plan and are configured per form, so leave requests and IT-adjacent onboarding tasks can land in different channels.
How many responses does the free plan cover?
500 a month, with unlimited forms, one seat and no card required. For a team of any size that is comfortable for leave and expense forms; a company-wide engagement survey is what usually pushes past it. Pro is 5,000 responses and three seats at $14.99 a month.
