Agency Intake Forms: Choose by Project Stage
Choose an agency intake template by the decision ahead: discovery, content collection, review or handoff. Each links to a real editable form.
Start with the next decision
An initial conversation, a detailed brief and a delivery record need different information. Choose a form because its answers let a named person act. Do not send every questionnaire to every client or collect final production details before you have agreed the work.
This collection follows an agency project through its stages. The marketing category remains the broader library; this page helps decide which form to use next and which records should remain separate.

Discovery and production briefs
Define a website's goals, page scope, content ownership and launch requirements before design begins.
Collect positioning, audience, brand values and visual references before a brand strategy workshop.
Brief a writer on the reader, desired action, voice, evidence and review process for a specific deliverable.
Set a video's purpose, runtime, production scope, location needs and asset permissions before scheduling a shoot.
Example: a website project with late content
A consultancy agrees a services website after completing discovery. Later, it supplies approved copy for one page and a draft for another. Use separate content-collection responses so each package has a clear version and readiness state. Sending the discovery questionnaire again would obscure the actual dependency.
If the client then asks for a resource library outside the agreed scope, record that as a change request. The project manager can assess the proposal without implying that receiving it approves new work. A revision to a sentence on an existing page is a different kind of request and can use the revision form.
Delivery, change and handoff
Collect one page's copy, image links, approval state and content owner for a website build.
Anchor design feedback to a version, explain the requested change and identify the next reviewer.
Record a proposed scope change, its business reason and the impact questions that need a decision.
Package final deliverables, transfer responsibilities, outstanding work and training needs into a client handoff record.
Choose the form layout for the work
Conversational discovery helps present one question at a time. A document brief lets the client read context and related questions together. Single-page requests suit a focused review item. These templates support all three layouts; the recommended default is a starting design, not a separate product or a restriction.
Before sharing, edit the choices to match your services, check required fields and test both sides of any branch. Identify who receives the answers and what the ending should promise. The template supplies a structure; it does not agree a contract, assign staff or deliver client work.
Keep the record connected
Use a stable project reference across stages. In a receiving sheet or your normal project system, link each response to its review decision and source version. A new form response should not silently overwrite an earlier approved brief.
At handoff, separate files delivered from access transferred and final acceptance. Give unresolved tasks an owner. That distinction lets the receiving team understand what is ready and what still needs action, even when the original project contact is unavailable.
Questions people ask
Should I use all these forms?
No. Choose the few stages your project needs. A small revision may need only one focused request, while a larger project benefits from separate discovery and delivery records.
Are these real templates?
Yes. Each linked template has a live preview and can be instantiated in the builder with editable fields, design and supported logic.
Does this replace the marketing category?
No. The category is a broader inventory. This collection explains which form serves a particular project decision.
