What makes these forms work
Branch on the answer immediately
Someone who cannot attend should see a short note field and a submit button, not four questions about meal preferences. One conditional rule on the first question halves the form for half your guests.
Handle the plus-one properly
Ask whether they are bringing someone, then reveal the name and meal choice only if they say yes. Guessing at plus-ones is the most common reason an RSVP headcount does not match the room.
