What makes these forms work
Break consent into the things being consented to
One checkbox covering six unrelated permissions is weak consent and everybody knows it. Separate the photography from the medical treatment from the data retention, and let people agree to each.
Capture the signature in the form
The e-signature question type captures a signature as part of the response on the Pro plan, so there is no separate document to send, chase and file. It records a signature on a submission — it does not generate a countersigned PDF, and if you need that, a document tool belongs downstream.
Record who consented and when
The submission timestamp is stored with every response. Note that if you switch the anonymous responses setting on, you are deliberately removing the identifying metadata — which is right for a survey and wrong for a waiver. Do not use it here.

Is the e-signature legally binding?
That depends on your jurisdiction and the kind of document, and it is not something we can answer for you — check with a lawyer for anything consequential. Is Zunoform HIPAA compliant for medical consent?
No. Zunoform does not offer a HIPAA Business Associate Agreement, and we would rather say so plainly than let you find out during a compliance review.
