What makes these forms work
Rate against described levels, not adjectives
"Exceeds expectations" means whatever the reviewer wants it to. Describe what each level looks like in the question or its description, and two managers rating the same person land closer together.
Separate the evidence from the score
Ask for an example alongside every rating. It slows the reviewer down in the right way and gives the person being reviewed something to act on other than a number.
Make peer feedback genuinely anonymous or do not call it that
