Feedback form templates

Feedback forms fail in a predictable way: they arrive too late and ask too much, so the only people who finish them are the furious and the delighted. These templates are short enough to complete in the moment, with the measurable question first.

What makes these forms work

Put the score before the essay

Ask the rating first and the open-ended "why" second. Someone who abandons after one question has still left you a number, and someone who continues has already committed to answering.

Follow up differently depending on the answer

A low score deserves "what went wrong", a high score deserves "can we quote you". Conditional logic makes that one form rather than two, and it is free.

Ask while the experience is still fresh

Feedback collected a week later measures memory. Trigger the form from the confirmation email, the ticket close or the end of the session — not from a monthly send.

Questions people ask

Can I collect feedback anonymously?

Yes, and properly. The anonymous responses setting stores no IP address, user agent, referrer, geolocation, language or screen size, and strips those identifiers from partial rows as well. Most builders describe a form as anonymous while still logging the metadata; this actually does not collect it.

How many questions should a feedback form have?

One rating, one thing that went well, one thing to improve is a complete feedback form. Add a question only when a specific decision depends on the answer — every extra one costs completions, and an unfinished form tells you nothing.
