Request form templates

Internal request forms exist to stop work arriving as an ad-hoc message. Their real job is routing: getting the right detail to the right person without a follow-up conversation. These templates ask the questions whoever fulfils the request would otherwise have to chase.

What makes these forms work

Ask for what the fulfiller needs, not what the requester wants to say

Write the form from the perspective of whoever has to act on it. If IT always asks "which device?", that is a required field, not something to discover in a reply thread three hours later.

Route by answer, not by inbox

A dropdown for the department or request type, plus conditional logic, turns one form into several queues. Add a Slack notification on Pro and the right channel sees it without anyone forwarding.

Set the expectation on the form

State the turnaround time where people can see it before they submit. Most chasing messages are not impatience — they are uncertainty about whether the request landed at all.

Questions people ask

Can requests notify the right team automatically?

Yes. Email notifications are on the free plan and can go to you, your team or the requester. Slack is on the $14.99 Pro plan and the channel is configured per form, so support, facilities and IT requests can each land where the right people already are.

Can I use one form for several request types?

Yes, and it is usually the better design. Ask the request type first, then branch with conditional logic so each requester answers only their questions. Conditional logic is on every plan including free, and you get one response table instead of six forms to maintain.
